Formerly known as Charleston House, Beaver Dam Assisted Living and Memory Care Communities provide assistance to mature adults with Activities of Daily Living. Medication Management, Incontinence Assistance, Bathing, Grooming, Dressing, Diabetes Management. We provide 3 meals per day as well as snacks. We have a robust activities program and go on weekly outings on our bus. We also provide transportation for medical appointments within 15 miles of our location. Our staff consists of cooks, laundry aide, housekeeping, activities, maintenance, bus driver and much more. Our CBRF serves individuals with advanced care needs (two person assists, sit to stand, hoyer lifts) as well as those with Alzheimer's/Dementia.
